Transplant Coordinator - Living Donation

Nurse Coordinator is a competent experienced nurse who consistently demonstrates a commitment to expand his/her knowledge and skill base, works collaboratively with interprofessional healthcare and quality improvement teams in a certain disease group.The Nurse Coordinator is responsible for coordination of the evaluation and preparation of patients within the relevant disease group (example: pre and post organ transplant, pre and post stem cell transplant, etc…). The Coordinator is responsible for coordinating patient care across multiple areas and levels of care, the outpatient clinic, the inpatient nursing unit and the community in order to provide continuity of patient care across settings.PRIMARY JOB DUTIES AND RESPONSIBILITIESPATIENT CARERecognizes abnormal, pertinent findings, and initiates referrals/consults accordinglyConduct pre-transplant interviews with prospective transplant patients and families. Interpret eligibility criteria for patients, families, physicians and outside agenciesPrepare and analyze pre-transplant clinical data and collaborate with other medical personnel in preparation of patients and families for transplantationEnsure patient and family education in all aspects of transplantationCompose written transplant summaries and clinic visitsCorrdinatepost-discharge planning and education for transplant patientsCompile and review post-discharge data for clinical management of transplant patientsProvide direct care to transplant patients in the Outpatient Transplant ClinicCollaborate with medical faculty, other transplant coordinators, nursing personnel, case management, social workers , transplant pharmacists and transplant dietician and other health disciplines to facilitate meeting patient and family needs. Coordinate and lead the transplant selection meetings and document the decisions made by the teamProvide on-call service on evenings, weekends and holidays when the transplant Services and Clinics are closedResponsible for completing charge capture education and in-service training for their Institute within CCAD. When charges are not captured automatically the nurse will complete within Epic in real time, Perioperative Services within 24 business hoursPROGRAM DEVELOPMENTCollaborate in the development of protocols and guidelines for patient managementDevelop, implement and evaluate comprehensive patient education programsParticipate in the development and execution of quality assurance programs and projectsPartners with other disciplines to enhance patient care through interprofessional education, consultation, management, technology development, or research activitiesAssure that the Cleveland Clinic Abu Dhabi Organ Transplant Program remains in compliance with the Department of Health Abu Dhabi, Federal Government and international standardsRESEARCHSupport planning of and participate in clinical research projectsCollect and analyze data for ongoing clinical research projectsIncorporate research findings into the Organ Transplant Program practicePROFESSIONAL DEVELOPMENT / EFFECTIVENESSStarts and pursues the journey of becoming a certified clinical transplant coordinator, by the American Board Transplant coordinatorSuccessfully completes Transplant Coordinator orientationMaintains up to date knowledge of transplant regulatory policies and procedures.
