Experience in chronic disease management, patient education, and digital tools is a plus.Key ResponsibilitiesPatient Care Support:Provide direct patient care including vital sign checks, blood glucose monitoring, and patient observations.Educate patients on diabetes management including lifestyle tips and monitoring instructions, ensuring compliance with care plans.Clinical Procedures Assistance:Prepare examination rooms and medical equipment, ensuring cleanliness and adherence to infection control standards.Assist in the preparation and handling of equipment for procedures, wound care, and specimen collection.Patient Education and Communication:Communicate effectively with patients, providing empathy and clarity to address their questions and concerns.Reinforce health education provided by physicians, encouraging self‑care practices and adherence to treatment plans.Documentation and Reporting:Accurately record and update patient information, treatment plans, and observations in electronic medical records (EMR).Report any changes in patient condition to the supervising nurse or physician promptly.Operational and Safety Compliance:Adhere to all DHA regulations, nursing guidelines, and GluCare protocols to ensure the safety and wellbeing of patients and staff.Participate in ongoing training and development programs to stay current with best practices in diabetes care and nursing support.RequirementsQualifications:Valid DHA Nurse License – A MUSTMinimum of 12 years of experience in a clinical setting, preferably in endocrinology, diabetes care, or chronic disease management.Knowledge of diabetes management and patient education in lifestyle‑related chronic conditions is preferred.Strong communication and interpersonal skills with a patient‑centric approach.Basic proficiency in electronic medical records (EMR) systems is preferred.Skills and Competencies:Compassionate and empathetic demeanor with a commitment to patient care.Ability to work effectively in a multidisciplinary team environment.Good organizational and time‑management skills.Strong attention to detail and adherence to clinical protocols.#J-18808-Ljbffr
