Job Description
Family Medicine Registrar will provide comprehensive primary healthcare physically and through telemedicine to individuals and families of all ages, encompassing preventive, acute, and chronic care. They will play a crucial role in diagnosing and treating a wide range of medical conditions, promoting health education and preventive measures, and coordinating care with specialists and other healthcare professionals. Additionally, they will be responsible for practicing population health management and value-based care, focusing on improving patient outcomes through efficient and effective healthcare delivery through specific care programs and clinical pathways, with an emphasis on delivering patient‑centered care and optimizing health outcomes.Patient Consultations and ManagementConduct comprehensive telehealth consultations and in-person consultations via thorough patient history and physical examinations with patients experiencing Family Medicine and general medicine complaintsProvide healthcare to all family medicine target population across age and genderProvide chronic diseases and preventive care consultations and follow up to patients as per clinical guidelinesUtilize video conferencing platforms to assess patient symptoms, review medical history, and perform virtual examinations. Develop personalized treatment plans, prescribe medications, and provide ongoing patient educationDeliver high-quality medical consultations to patientsUtilize telehealth platforms for virtual appointments and conduct in-person assessments as neededDevelop and implement personalized treatment plans, including medication management, referrals to specialists, and lifestyle modificationsOversee ongoing care for chronic and acute conditionsAdjust treatment plans as necessary based on patient feedback and clinical outcomesCollaborate with other healthcare professionals, including primary care physicians, specialists, and nurses, to ensure seamless patient carePractice population health management by identifying and addressing health issues at the community level, implementing preventive measures, and promoting health education and awarenessDocument findings and treatment plans meticulously in patient recordsQuality Assurance and ComplianceCommunicate effectively with patients and their families, providing clear and concise explanations of diagnoses, treatment options, and follow‑up careAdhere to all relevant medical regulations, ethical guidelines, and quality assurance standards for telehealth practice and in-person practiceMaintain up-to-date knowledge of telehealth regulations and best practices. Stay abreast of advancements in family medicine, in‑person care and telehealth technologyStay up to date on the latest advancements in family medicine and telehealth technologyTelehealth Platform ProficiencyMaintain accurate and complete patient records in accordance with HIPAA regulationsMaintain proficiency in utilizing the designated telehealth platform, including video conferencing, secure messaging, and electronic health records (EHR) integrationActively participate in training programs and stay updated on platform enhancements and best practices.
